What you describe below is how I did my shot this morning. Motion tracked, then hand-finessed by setting occasional anchor point keys. Turned out awesome. Quite pleased with myself. ; )  Exported patch layer as AVI+Alpha and overlayed the clip in Pr. I set the patch layer to be a 3D layer and turned on motion blurring. The motion blurring of my patch.png file matches the original clips shutter angle set at 180° .

Yeah, sometimes hand tracking is all you can do. I've been using Command-Left and Command Right and just the arrow keys themselves lately to do the manual tracking without having to lift my fingers off the keyboard. Being able to quickly flip back and forth between frames means I can pretty quickly see whether a point is locked without having to do averaging. I also have a habit of flipping forward a few frames to see where the motion of an object shifts. Let's me get away with only setting track points every few frames sometimes.
