I sussed the issue on my Mac to the persistent disk cache and OS 10.8. = I defeated it by disabling Disk Cache, or deleting the cached files = whenever I encountered the issue. Since it takes 5 minutes to delete = 40-50G from my HD, that wasn't acceptable, nor was disabling one of the = coolest new CS6 features, I reverted to using Lion, where I haven't been = vexed by it since. I don't know if this is helpful, since you said you flushed your cache = to no effect, but I'm not sure if your bile is directed at the right = perpetrator.
