| Your issue sounds like the one I was having as explained in a thread titled "weird" on 8/3/2012.
I sussed the issue on my Mac to the persistent disk cache and OS 10.8. I defeated it by disabling Disk Cache, or deleting the cached files whenever I encountered the issue. Since it takes 5 minutes to delete 40-50G from my HD, that wasn't acceptable, nor was disabling one of the coolest new CS6 features, I reverted to using Lion, where I haven't been vexed by it since.
I don't know if this is helpful, since you said you flushed your cache to no effect, but I'm not sure if your bile is directed at the right perpetrator. I first noticed it on plug-ins, and came to a similar conclusion, but I also saw this behavior on footage with no plugs applied, and changed my theory.

Under the same assumption, I'm inclined to concur. Flush the crappy disk cache.
