AFAIK, Nuance is ahead of the game here. Check out their Dragon Dictate 3 for Mac. In addition to standard voice recognition, it includes features to control common Mac apps such as Word, Excel, Keynote, Mail, etc. And there's now an "Express Editor" feature that lets you dictate text into a text field of any (or at least many) Mac apps.

I don't use Dragon, but I have friends who, because of various CTD/RSI issues can only use a keyboard a little bit or not at all. They all use Dragon and are fairly productive. Don't personally know of anyone using it with AE, though.
