I always thought wrists were the thing to watch out for, however I am finding that my shoulders are more sore than my wrists and my knees get so stiff I walk like John Wayne whenever I get up after even an hour or so of sitting. Add to that all the research about sitting too much and I think an adjustable desk is in my future.

Using the Wacom rocker switch as Right and Middle mouse clicks pus the tap for left click is what I've been doing for 14 years.  I agree Emile that the click drag is a stressor. I have been considering some kind of foot pedal for use as a modifier key. I'll have to look at the Elite.

Also, I never thought I would get any RSI from using a tablet but I feel some issues going on near my elbow. It took over a decade but I think using the stylus may have eventually caught up with me. Lately I'm using a mouse about 25-30% of the time to switch it up. I'm also considering a larger tablet to shift some of the movement to my shoulder instead of all wrist and elbow.

I found that by far the most stressful action was hold and drag. I got an Elite foot pedal to mimic the left mouse button and have found it very helpful.

I've used a wacom for years. I use the keyboard, however, for as much as possible, so I don't miss the third button much at all. I have a (3 button) mouse connected for those times I need that functionality (say in Color). You can get wacoms with scroll wheels and track pads, etc.

Over the years I've used mice, trackballs, and tablets but always in conjunction with the keyboard. I try to vary it as much as possible. If I'm doing metadata intensive or repetitive tasks I often switch to the mouse.

I'm lucky in that I vary rooms, so my desk height changes subtly, and I use my laptop and desktop systems at the same time, so my hand positions change.

A good heavy coffee cup helps, too. A squeeze ball, a dumbbell, maybe even a second mouse of a different size. It all seems to relate to variation of your body/hand position.

HTH, and good luck with it,

sorry to bring up this topic again, but I recently have been experiencing some RSI symptoms and I have a couple questions:

 - to the people who have simply switched mouse hands (right <> left), how do you deal with your keyboard? Do you still use the modifier keys on the left side, or do you switch to the right? if you switch to the right, what do you do about undo (cmd/cntr - Z) copy, paste, select all, redo etc... ?

 - to those who use a tablet, do you find you still have the same functionality as your 3-button mouse? I rely heavily on mouse buttons, especially for general camera navigation in C4D, and am concerned because the stylus only appears to have 2 buttons. Also, no more scroll-wheel, correct?
