It's still a requirement on most deliverables for distributors. The last two movies I worked on had a requirement for a 4:3 pan and scan. You have to think outside your own backyard to places like India where much of the viewing public still consumes 4:3 images, but hey someone is paying money for this format, so bring it on. 

I actually find it fun doing 4:3 pan and scans from a widescreen master. You have to get quite creative about how you tell the story at certain points in the film where two subjects are on either side of the screen.
