Using CS6?  Usually the first thing I suggest our artists try when things like this happen is to clear the disk cache.  AE > Preferences > Media & Disk Cache > Empty Disk Cache.

... I've seen watermarked frames from unregistered plugins persist in the cache, fonts that weren't activated, etc. etc.  And if AE doesn't think it needs to update that frame in the disk cache, it will persist between sessions.

May not be your problem, but something to try!

Mike, I typically pull the key with keylight, set output as a matte, duplicate the layer and use it as a luma matte. This kills most noise and chatter. Maybe it would help with your frame issues?

i split the clip into layers using magnum script. also getting phantom masks but i think that might be because i went a bit copymask2layer crazy.
