The cache is written to the disk if takes longer than xx amount of time to render... or that is how i understand how it works. Someone at Adobe could describe how it works much better, Todd?
The problem with AE not overwriting wrong cached frames was much more common just when CS6 was released last year, and is mostly because thirdparty plugins havn't been updated to take care of the disk cache properly. Some times you can work around it by changing some parameter that has no effect on the effect (!) and that will force an update, but sometimes you have to either disable the diskcache (and restart AE) or in worst case, delete ALL your diskcache. 
But most plugins have been updated to play nicely with the diskcache, so i see it more and more rarely, just make sure you have the latest updates installed.

Regarding the SSD disk cache, I assume cache is saved on the SSD for any rendered frame (just as on a spinning HD) as long as one doesn’t go into settings and clean the disk cache?
 
Is it common that AE caches 1 frame wrong like that?

And i will agree with all above. Get as much RAM as possible for the amount of money you want to spend, it should be fairly cheap to put in 48GB, and a SSD to the disk cache. You will not want to turn back to the old setup when you start working with AE with huge amounts of RAM and a fast SSD :-)
 
...only thing is that cleaning the SSD disk cache is a all or nothing, you will have to loose 500GB perfectly fine disk cache because of 1 frame that AE cached wrong :-(

RAM and SSD make a really big difference especially if you do multi-proc rendering.
