You choose a folder or folders to share and you can give someone either a key for full bidirectional syncing or for one-way syncing (aka read-only or a backup), and can also give them a key that only works for 24 hours. They install the software, select a folder, and put in your key. That's it. Of course both machines have to be on at the same time. It will connect any two machines anywhere in the world, using the shortest and quickest path possible (it's based on Bittorrent algorithms, after all). There are servers that will be used to connect the two if they can't get to each other directly, and you can turn that off if you want for privacy/security.
