Isn't real time playback kind of the point of using Premiere?  Is your media storage fast enough for what you're trying to play?

I know it's hard to use an editor for perfect reviewing at normal speed but sometimes it plays fairly smooth, others not and never consistently.

I have zero effects going either.  These are tracks rendered out of AE for final render in Premiere. Very hard to get full feel for timing in AE.
