You are going to need to give us a lot more information about your system and your project, if you want us to give you a feedback on why your are not able to playback realtime in Premiere. For a start, file format? codec? frame rate? internal or external HD? RAID array? video card?...

For you information all of my HD projects play realtime in Premiere Pro...

Is there a way to have Premiere Pro sort of do a RAM preview so that your project plays at least reasonably well.

I know it's hard to use an editor for perfect reviewing at normal speed but sometimes it plays fairly smooth, others not and never consistently.

I have zero effects going either.  These are tracks rendered out of AE for final render in Premiere. Very hard to get full feel for timing in AE.
