Not your error but I found when making a DCP for a client that could not render on their end and they sent the whole project drive to me, that Premiere (C6 through latest CC) is prone to severe memory links when relinking and importing footage on very large projects. You might doing some testing to see if memory issues are present.

Also for all - It's not an specific file and seems very random.  One file may work once, then not again or one that didn't import the first time may do so on a second try.
