If you just want to extract the pixels from raster images in a pdf, then photoshop can do that if you just set the import dialog tp Images instead of Pages.

Just played around and learned Acrobat (Pro) has a Save as EPS option.  It made a PDF with vector and bitmap into two documents; one with the vector, and another with the bitmap.

Well… I just got a Word document from a client with a bunch of images she wants me to put in a video, and the same tip works on Word documents.

And they're at their original resolution, and not downscaled to fit on a sheet of letter paper.  Pretty cool!
