Unexpected and bizarre behavior in Pr is corrupted prefs 99% of the time (in my experience since CS 5.5). Launch the app with the option/alt key pressed.
Other routines: delete all caches and previews.
Corrupted sequence is another possibility. Create a new Sequence. Copy the old one and paste into the new one.
Corrupted project is another possibility. Create a new project, and import the old one, or drag everything from the “bad” project to the new one.
Uninstall and reinstall the app if all else fails.
I see two instances of Ultra Key in your screen grab with the Effects tab. Could that have something to do with your issue?
